IMPROVE FIRE SAFETY AND DONATE FUNDS
Contributions to the foundation can be made to the Survive
Alive Foundation, which supports the Chicago Fire Department’s
Public Education efforts. All contributions can be sent to
the Director of Public Education, located at the Survive Alive
House, 1010 S. Clinton Street, Chicago, IL 60607. To contribute
to the SAHF, complete the Sponsorship
Commitment Letter and forward it as directed on the form.
With larger contributions, the charitable party may designate
a program for those funds to be allocated to a specific use,
such as purchase and distribution of smoke detectors, coloring
books, or other promotional fire safety educational materials.
With significant contributions the Survive Alive Foundation
and the Chicago Fire Department will make an effort to allow
the charitable party to publicize their work – this
is only done at the discretion of the contributor.
